Kids love to play games. Only parents don't like to play kid's games since with most of them, you have to lay low or the kid can't win.
I think if a parent (or any adult) has no problem in winning from a kid, the game is too hard for the kid. And that's what's fun with Operation. Kids have a steady hand but go all nervous on this game whereas older people just don't have the steady hand. This makes Operation fun to play, both for the adult and the kid. It takes 15 minutes to play and is great fun even if it's not your turn.
So why only three stars? Well, for every item there is a trick to take it out without much risk. And once you know these, the game loses it's hookability. But until then, it's a great parent-kid play.
